General

Chronic Care Management (CCM) is monthly, non-face-to-face care coordination for patients with two or more chronic conditions. Remote Patient Monitoring (RPM) tracks physiologic data — vitals like blood pressure or glucose — captured by a connected device. Remote Therapeutic Monitoring (RTM) tracks non-physiologic, patient-reported data, such as therapy adherence or pain levels. Many patients qualify for more than one program at the same time.

Yes. CCM, RPM, and RTM can be layered for the same patient when clinically appropriate and properly documented — for example, a CHF patient might receive CCM for overall care coordination and RPM for daily weight and blood pressure tracking.

Yes. Every CHMS program is built around CMS's documentation, time-tracking, and consent requirements for CCM, RPM, and RTM billing.

Eligibility & Consent

Medicare patients with two or more chronic conditions expected to last at least 12 months, or until the death of the patient, generally qualify for CCM.

Patients with a chronic or acute condition that can be tracked through a remote monitoring device — such as hypertension, diabetes, or CHF — and who consent to using the device are generally eligible for RPM.

Yes. Patients (or their authorized representative) must provide consent before enrolling in CCM, RPM, or RTM, and that consent must be documented.

Getting Started

Timelines vary by practice or community size, but most CHMS partners are enrolling their first patients within a few weeks of onboarding.

It depends on the program. RPM typically involves a connected device (see our Devices We Use page), while CCM requires no new hardware. We'll walk through what fits your existing workflow during onboarding.
