Devices We Use

Devices We Use

The hardware and software behind CHMS's Remote Patient Monitoring and Remote Therapeutic Monitoring programs — built for reliability, ease of use, and consistent monthly data.

Remote Patient Monitoring

Vitals Monitoring Stations

For Long Term Care and Assisted Living communities, CHMS deploys a multi-parameter vitals monitoring device that captures blood pressure, pulse oximetry, resting heart rate, and respiratory rate in one device — giving nursing staff a single, simple workflow for daily rounds.

Home & Practice-Based RPM Devices

For practices and home-based patients, CHMS supports standard connected devices matched to each patient's monitored conditions:

Blood Pressure

Systolic/diastolic readings captured during daily rounds, used for hypertension and cardiovascular monitoring.

Pulse Oximetry (SpO₂)

Tracks blood oxygen saturation — commonly used for COPD, CHF, and respiratory conditions.

Resting Heart Rate

Daily pulse tracking to catch early signs of cardiac strain or arrhythmia.

Respiratory Rate

Breathing rate monitoring, an early indicator of respiratory or cardiac decline.

Remote Therapeutic Monitoring

App-Based Tools for Therapy & Symptom Tracking

RTM tracks non-physiologic, patient-reported data — how well a patient follows a treatment plan between visits. CHMS's approach centers on a simple mobile experience rather than additional hardware.

CHMS Care App

A patient-facing mobile app for daily check-ins, symptom self-report, and therapy adherence tracking.

Symptom & Pain Tracking

Structured daily or weekly questionnaires tailored to each patient's condition and care pathway.

Therapy Adherence Logging

Tracks completion of home exercise programs, such as pelvic floor therapy or post-surgical rehab routines.

Chronic Care Management

No New Hardware Required

CCM is built around care coordination, not devices — medication reconciliation, care plan updates, and monthly outreach. It layers directly on top of whatever RPM or RTM devices a patient already uses, without adding anything new to manage.
