How do our services work?

  • CHMS team members work with the Medical Director /Facility Team to implement services through the following:

    • Education to Facility of CCM and RPM Programs ​

    • Education to POA's, Families, and Residents of CCM and RPM Programs

  • CHMS provides CCM services by performing the following:

    • Assist providers in obtaining consents for each qualifying resident ​

    • Collaborate with Facility Care Team to provide monthly monitoring services  

    • Communicate with Medical Director and Care Team any changes/flagged alert that attention

    • Document monthly CCM requirements in EHR 

    • Drop claims for each month's services through Medical Directors billing system

  • CHMS provides RPM services by performing the following:

    • Assist providers in obtaining consents for each qualifying resident

    • Provide, install, and educate Facility Care Team on the utilization of RPM monitoring devices

    • Collaborate with Facility Care Team to provide monthly monitoring services for each resident 

    • Communicate with Medical Director and Care Team changes/flagged alerts requiring attention

    • Document monthly RPM requirements in EHR 

    • Drop claims for each month's services through Medical Directors billing system

What is the Revenue Opportunity for Providers?

  • CHMS completes all of the work monthly and drops the claim

  • Medical Director reviews documentation and signs off on monthly billable services
